Service Description

This is an adoption medical. Before booking, please confirm that your organisation would accept a form from our clinic. Before booking a copy of your full medical summary would be required, which can be obtained from your NHS GP, including where relevant, any recent blood test, scans or hospital/consultant appointments. You would also need to fill in your part of the form (please fill electronically if possible) prior to the appointment and email this to us. The completed form can be sent via email.
