Before booking, you will need a copy of your medical form and your medical history summary (which you can request by contacting your surgery). Please ensure to send these to us or bring them to your appointment. This is so we can confirm your medical history as is required on the medical form. 

 

Once you have these you can book an appointment anytime. A telephone or clinic appointment can be booked for this medical. If booking a telephone appointment, the completed form can be emailed to you after your appointment.
