Driving/Taxi/HGV/D4 medicals are available at our clinic.

If you wear glasses/contact lenses, please bring a copy of your prescription strength to your appointment. For certain visual conditions, you may be advised to have your eye test completed with an optician.

Before booking a copy of your 'Patient Summary' would be required - this is a short document containing your medical history, which can be obtained from your NHS GP Surgery. Alternatively, access to your medical records via an NHS app would suffice, but only if full records are available.

 

Please bring along any additional information if needed e.g a list of your medications or the names of your consultants/specialists if you are under any consultant care.
